If you have received a red card at the facility you MUST fill out this form and either bring it in to give to a manager or email it to info@lasvegasindoorsoccer.com

You will not be allowed to play until this form is filled out. You will serve a minimum 1 week suspension from all leagues, then the length of suspension will be determined by management.

Multiple red cards within a short period of time will result in a 6 month to 1 year ban.
